In some cases, patients may be discharged from the service if specialist input is no longer required – but they will always have the option to be re-referred to the service if their needs change.
What to expect
When we first meet with a patient and their family, we carry out a comprehensive assessment-not only of the patient’s needs but also of the support required by family members or carers. Our approach is collaborative, empathetic, and fully person-centred.
