How to be referred
To ensure individuals receive the most appropriate care at the right time, all referrals must be made by a healthcare professional.
This includes GPs, hospital teams, community (district) nurses, or members of our palliative care teams.
If you or someone you care for may benefit from hospice care, please speak to your GP, hospital doctor, nurse, or another healthcare provider. They will be able to assess your needs and, if appropriate, submit a referral to our service on your behalf.
We aim to ensure timely responses to referrals and work closely with other care providers to provide coordinated, high-quality support, whether in our inpatient unit, the community, the hospital, or via our outpatient and therapy services.
Who can be referred?
Referrals may be appropriate for:
-
Patients with complex symptoms as a result of a life-limiting illness
-
Patients with specialist end of life care needs
-
Family members, including children and young people
-
Friends or carers involved in the care journey
-
Anyone affected by the death of someone under our care
